- Go to Communication > Surveys > Configuration > Form Fields
- Click on the Add Field button
- Enter a (general) name for the form field
- Select the field type Rating scale
- Choose the desired number of rating options
- Optionally, select to display the option not applicable (n/a)
- Click on for the valuation option
- Enter the value (for example ‘1’ or ‘very good’) and click on the Save and close button
- Repeat steps 7 and 8 for each option
- Click on the Add button
- Enter the question you want to ask and click on the Save and Close button
- Repeat steps 10 and 11 for each question you want to ask
- Click on the Save and Close button
Practical tip
Use your grades as rating options, then clarify what a grade stands for. For example, state: “Your opinion is important to us. Indicates which grade applies (1 = unsatisfactory / 10 = excellent)'.
